Emergency Action Plan
Pre-Participation Forms and Physicals
All athletes must complete and turn in pre-participation forms and physical exam form into the sports medicine department by July 15th each year. We ask that each student athlete have a physical exam by their own physician before arriving on campus. A COMPLETED packet must be received in order for you to receive clearance for athletic participation from Lee University Athletics.
Insurance explanation/recommendations
It is the recommendation of the athletic department that each athlete have a primary medical insurance policy. The Lee University Athletic Department provides a secondary insurance for athletic injuries only. For an athletic injury, the student-athlete's primary insurance will be billed first and any excess will be taken care of by the athletic department.
It is for this reason that we request that each student athlete be covered under a primary medical insurance policy for all non-athletic medical care.
If the student-athlete receives a bill pertaining to an athletic injury, please turn the bill in with an explanation of benefits (from primary insurance) to Tena Walker. Any bills, EOBs, or payment questions should be forwarded to:
